Forms and Documents

For each child you would like to enroll, please complete and sign the following forms:
1. NEW STUDENT REGISTRATION FORM (2 page document)
2. ENROLLMENT FORM (3 page document)
3. MEDICAL STATEMENT (1 page document & must be signed by a physician).
Please drop all forms with Registration Fee in the large BLACK MAILBOX at the rear of Trinity United Church of Christ located at 215 High Street, Wadsworth, Ohio 44281 During BREAKS and over the SUMMER please TEXT 330-414-9841 to ALERT us that you have dropped off a registration!!!
- All pages of the forms must be completed in full, along with a signature of parent or legal guardian, for names to be officially added to the registration list.
Sign-ups are on a first come, first serve basis.
- Signing parent or legal guardian are not required at day of registration. You may send them with a third party, as long as all forms are fully completed and signed.
Non-Refundable Registration Fee of $100.00 must be submitted with each registration completed.
- Please make checks payable to Grizzly Club Preschool
The Medical Statements are due AT OPEN HOUSE

How to Register over the Summer:

Print ALL the forms: Enrollment, Registration and Medical Forms
Fill out forms completely (medicals must be sign by physician)
Drop forms and $100 Registration fee in the large Black mailbox at the rear of Trinity Church ( 215 High St, Wadsworth Ohio 44281) TEXT 330-414-9841 to ALERT us that it is there!
Do NOT mail to the school address, no one will see it until the fall and you will likely lose your spot! Check your EMAIL, we will email you when your packet is received to let you know your spot has been secured. If the class has filled, you will be offered a spot on the waiting list. Fee will be returned if a spot is unavailable.
Summer tours are NOT offered, please take the virtual tour on the Home page.
Please call 330-607-7551 with any questions you have and leave a message. Your call will be returned as soon as possible.

For a small fee, and FASTER registration...
Email Registration forms along with credit/debit card information
(card number, exp. date, zip code and 3 digit security code) to Grizzlyclubpreschool@gmail.com and $105 registration fee will be charged to your account upon receiving your registration.
